Abstract

Type 2 Diabetes Mellitus (T2DM) is a chronic disease that requires therapy to control blood glucose. Uncontrolled blood glucose can reduce the quality of life and increase medical costs. This study aimed to analyze differences in Random Blood Glucose (RBG) levels as the clinical outcome for three months based on the quality of life and direct medical costs. The clinical outcome was considered as controlled when RBG values <200 mg/dL, while RBG values ≥200 mg/dL was otherwise stated uncontrolled. This study was a cross-sectional with observational design. The inclusion criteria were T2DM patients taking the same antidiabetic at least for three months at the outpatient clinic of Panembahan Senopati Bantul Hospital in September 2017. Those who were pregnant or breastfeeding were excluded from the study. Data were collected through questionnaire filling, added by patients’ medical and financial records. Demographic data were analyzed descriptively, while clinical outcomes were analyzed using Kruskal-Wallis test. The study showed that most patients had uncontrolled clinical outcomes (64.5%). The average quality of life was 65.7±7.7, while the average direct medical cost was Rp 489.005. There were differences in clinical outcomes based on quality of life (p=0.000) at domains of physical function (p=0.034), personal satisfaction (p=0.000), treatment satisfaction (p=0.000), frequency of disease (p=0.012) and based on direct medical costs (p=0.012). The study suggests that patients with controlled clinical outcomes show a higher quality of life and lower direct medical costs.
